Required Memberships, Forms and Important Policies
USA Ultimate (USAU)
All players must have a current USA Ultimate membership/ID# before registering for the season. "Youth membership" is strongly advised (in order to be eligible for State Championship play as well as any event hosted by a non-USAU affiliate). "Affiliate Player" is an additional option at a lesser cost. However, please note, an upgrade will be required to participate in the events listed above. All memberships are valid for the calendar year (Jan thru Dec).
Returning players: login in to check your status &/or renew
Sign the Liability Waiver AND Infectious Diseases Waiver before the first day of practices (these can be found on the USAU website after you login and you do not need to print them). If these are incomplete, players will not be allowed to attend practice until they are completed.
All players must bring a signed Medical Authorization Formto practice with them on the first day and give it to their coaches. Under "Name of Chaperone" please list "NV Ultimate". This form is valid for one calendar year.
COVID-19 Impact on Play
NV Ultimate is closely monitoring state and local guidelines as we structure and plan play opportunities and has our players' safety at the forefront of all of our decision making.

Players who must miss a portion of the season due to positive testing for COVID-19 will be refunded 50% of playing fees (Registration less material costs of jerseys, mask & disc) if they are unable to attend 50% worth of practices/games due to illness. In the case where practices must be cancelled for multiple weeks due to several positive tests, an online coaching and training regimen will be included for all players.
A full refund will be given if the refund request is more than 30 days prior to the first day of the season or event/and jerseys have not been ordered. If personalized jerseys have been ordered, material costs (disc and jersey where applicable) will be deducted from the refund).
From 30 days up to the first day the season or event starts, half of the registration fee will be returned.
As of the first day of the season or event, no refund will be given.
Please note, we order jerseys and discs well in advance as well as hiring coaches/ reserving field space based on the amount of players we have registered. While trying to remain flexible, we need to cover our costs. We appreciate your understanding on this issue and thank you in advance for working with us. Any players interested in applying for a scholarship towards their registration and tournament fees, please fill out our Scholarship Application Form. Please note, requests must be submitted no later than one week prior to the first day of practice for consideration.
